Rapid quantitative method using spin columns to measure porphobilinogen in urine.

چکیده
BACKGROUND Large increases of urinary porphobilinogen (PBG) indicate acute porphyria, which may be due to acute intermittent porphyria, variegate porphyria, or hereditary coproporphyria. These conditions are relatively rare but share symptoms with more common conditions, such as acute surgical abdomen, and often must be ruled out rapidly. Reported quantitative methods for PBG measurement are time-consuming and inconvenient. We developed a rapid quantitative method that uses resin-packed spin columns to measure PBG in urine. METHOD We applied urine to anion exchange resin in a spin column, then performed centrifugal separation and washing. PBG was eluted in 1 mol/L acetic acid and reacted with Ehrlich's reagent. After 5 min, we measured absorbance at 525, 555, and 585 nm. PBG concentration (mg/L) was calculated as 88 (A(555) - (1/2)(A(525) + A(585))). RESULTS The reportable PBG concentration range was 0.2-15 mg/L. Between-day (total) imprecision (CV) was 8.4% at 1.2 mg/L and 3.5% at 4.4 mg/L. Comparison with our established method (x) yielded a Deming regression equation: y = 1.04x - 0.01 mg/L (R(2) = 0.98; S(y,x) = 0.87 mg/L). No interference was noted from urobilinogen or highly colored urine specimens. CONCLUSIONS This method for PBG measurement is more rapid and precise than other methods. This test can serve as a quick screening test and facilitates batch analysis for routine quantitative testing.

A Quantitative Assay for Urinary Porphobilinogen.
A procedure has been developed for the rapid, quantitative determination of urinary porphobilinogen. Interfering substances are removed by selectively adsorbing the porphobilinogen with an ion-exchange resin using a batch method. After acid elution of the porphobilinogen, Ehrlich’s reagent is added to produce the chromophore, which is then measured spectrophotometrically. Obtaining absorption a...

A rapid colorimetric method, sensitive to 0-5 ,tg., is described for the determination of delta amino-laevulinic acid in urine. The method consists of a modification of the picrate method of Shuster in which interference by porphobilinogen is avoided by the use of an ion exchange resin. The method is found to give results which compare in accuracy with the longer standard method of Mauzerall an...
متن کاملEvaluation of a commercially available rapid urinary porphobilinogen test.
BACKGROUND Demonstration of substantially increased urinary excretion of porphobilinogen is the cornerstone of diagnosing acute porphyria crisis. Because porphobilinogen testing is not implemented on clinical chemistry analysers, respective analyses are available in rather few clinical laboratories. The aim of this study was to critically describe and to evaluate a semi-quantitative rapid test ...
متن کاملRanitidine and high concentrations of phenylpropanolamine cross react in the EMIT monoclonal amphetamine/methamphetamine assay.
Toxicity of heavy metals and the genetic porphyrias (most commonly acute intermittent porphyria, AlP) produce increased concentrations of &-aminolevulinic acid (SALA) or porphobilinogen in plasma and urine. The quantitative test for S-ALA or porphobilinogen in urine described by Mauzerall and Granick (1) involves column separation and is recommended (2). For monitoring patients more closely, it...

The porphyrias are disorders of haem biosynthesis which present with acute neurovisceral attacks or disorders of sun-exposed skin. Acute attacks occur mainly in adults and comprise severe abdominal pain, nausea, vomiting, autonomic disturbance, central nervous system involvement and peripheral motor neuropathy. Cutaneous porphyrias can be acute or chronic presenting at various ages. Timely diag...
